Spinal seizures after metrizamide myelography in a patient with a spinal block.

چکیده
Since the introduction of the water-soluble contrast agent metrizamide, reports of side effects in man following myelography have been limited to the frequent occurrence of relatively minor symptoms such as headache and vomiting , and the occasional episode of grand mal convulsions [17]. Such convulsions may relate to an excessive quantity of the contrast agent spilling over the convexity of the brain and are sometimes facilitated by phenothiazine medication [8]. As a result , metrizamide has been considered safe for total myelography, c isternography , and ventriculography . We report a case of spinal seizures, in the form of prolonged myoclonic spasms of the trunk and lower extremities , occurring after metrizamide myelography in a patient with a myelographic block in the midthorac ic region.

Complete myelography with metrizamide.
Thorough myelography of the entire spinal cord including the foramen magnum region can be accomplished consistently and promptly after metrizamide injection via lumbar or lateral cervical puncture. When lumbar puncture is used the patient's torso is oblique, but the neck is prone and straight and the table is tilted 20 degrees head-down to allow direct cervical filling with contrast material. E...
متن کاملTransient Expressive ( Nonfluent ) Dysphasia after Metrizamide Myelography 945
Four (3.4%) of 117 patients undergoing metrizamide myelography experienced transient expressive dysphasia 7-8 hr after myelography and lasting up to 36 hr. All four patients had lumbar myelograms obtained with 15 ml of 190 mg I/ml (2850 mg I). Metrizamide was injected via lumbar puncture with a 20 gauge spinal needle under fluoroscopic control. Neurologic complications after metrizamide have be...
متن کاملTotal myelography with metrizamide through the lumbar route.
Myelography of the entire spinal cord was performed in 64 consecutive patients using metrizamide via lumbar puncture. Adequate to good opacification of the cervical and thoracic subarachnoid space was obtained in 63 patients. The technical factors responsible for this success rate are patient cooperation, high iodine concentration, high iodine concentration (250 mg/ml), slow injection rate, del...
متن کاملIopamidol vs. metrizamide myelography: clinical comparison of side effects.
One hundred myelographies with iopamidol and 100 with metrizamide were performed in order to compare the side effects of the two contrast media after injection into the spinal subarachnoid space. All patients were observed for a follow-up period of at least 4 days. The most frequently observed side effect, headache, was more common, of longer duration, and more severe with the use of metrizamid...
متن کاملMyelography in cancer patients: modified technique.
Because of the frequency of multiple sites of involvement, a thorough evaluation of the entire spinal canal is essential for detection and treatment of spinal compression in cancer patients. For the examination of the entire spinal canal in cancer patients, a modified approach is described using water-soluble contrast medium (metrizamide), oil-based material (Pantopaque), and a "squeezing" tech...
